patients only.
Please read about your two request service options below.
You will be asked to select an option on the next screen.
OPTION #1: EXPRESS SERVICE OPTION

MEDICAL RECORDS - EXPRESS

Processing fee: $10 flat fee per request.
There are no refunds for any reason.

All available Medical Records are provided.
You will not be asked to specify the types of Medical
Records, physicians, or dates of service.

Completion Time: Allow up to 48 hours
for your Medical Records request to be completed.

You will be texted or emailed when your requested Medical
Records are ready to access online
(see text samples below).

Medical Records will be made available securely
online only.
We do not fax, email, or mail records.
OPTION #2: STANDARD SERVICE OPTION

MEDICAL RECORDS - STANDARD

Processing fee: NO CHARGE
for the last two years to date.

You will receive any/all Medical Records
from the last two years to date.
You will not be asked to specify the types of
Medical Records, physicians, or dates of service.

Completion Time: Allow up to 7 days
for your Medical Records request to be completed.

You will be texted or emailed when your requested
Medical Records are ready to access online
(see text samples below).

Medical Records will be made available securely
online only.
We do not fax, email, or mail records.
Who Can Order Patient Records Online?
Only patients, or those individuals with the documented legal
authority to make healthcare/medical treatment decisions for the
patient, can use this ordering site to request medical records,
as described below:

Patients who have reached the age of majority: 18 years of age
or older
(NOTE: A parent may
not request
records on behalf of an adult child, regardless of insurance
coverage, residence, etc.).

Parents/guardians who have the legal authority to access a
minor child's records.

A person who has the documented legal authority to make
healthcare/medical treatment decisions for a patient of any
age (healthcare conservator, medical/healthcare POA,
legal guardian, executor/administrator of estate).
What You Need to Get Started

The patient's valid driver's license or other valid
government-issued ID.

If you are the parent/guardian of a minor patient or have the
legal authority to make
healthcare/medical treatment decisions for a patient
of any age, you will need to upload your valid ID as well.

A credit card to make payment, if applicable.

If you have the documented legal authority to make
healthcare/medical treatment decisions for a patient,
you must also upload official, currently valid
documentation of this authority
(Durable POA for Healthcare, Medical POA, Advanced Directive for
patient and designated caregiver; Death Certificate for
deceased patient).
NOTE: Financial POA is not sufficient.
If you cannot provide both your and the patient's ID as well as
documented proof of your legal right to make healthcare/medical
treatment decisions for the patient, the patient must
place his or her own request.
Please have these items handy before you start!
